TERMS AND CONDITIONS

Subscribers can earn Referral Rewards if: (i) a referred colleague clicks on their referral link to create a valid Vitality Bone account; and (ii) the referred colleague subscribes to Vitality Bone and apply the referral promo code. (iii)Both parties will receive the Referral Rewards once the referred colleague completes 2nd order with Vitality Bone subscription.

A subscription order will not be qualified for Referral Rewards if:

• The referring subscriber and referred colleague have the same account number.

• The referring subscriber and referred colleague have the same shipping address, or same NPI number.

• The referred colleague cancels his/her subscription order before completing the second order with his/her subscription.

• The referred colleague or the referring subscriber is not a licensed clinician.

• The referred colleague is already an existing customer.

Multiple Referrals

• A referred colleague may only use one referral link. If a referred colleague receives referral links from multiple referring subscribers, rewards will only go to one specific referring colleague upon his/her choice.

• The Referral Program may not be combined with other Vitality Bone promotions or incentives.

Termination and Changes

• Vitality Bone may suspend or terminate the referral program or a user’s ability to participate in it at any time for any reason.

• We reserve the right to suspend accounts or remove referral rewards if we notice any activity that we believe is abusive, fraudulent, or in violation of the Vitality Bone Terms and Conditions.

• The scope, variety, and type of services and products that you may obtain by redeeming referral rewards can change at any time
